The sworn statement according to HG 843/1999 regarding the type of Public Food Service Unit - The document must be submitted in ORIGINAL.*

According to Order MFP 1329/2004 at the Public Finance Administration*

Copy of the Identity Card or Identification Bulletin and a Copy of the Unique Registration Code at the Trade Register, including the annexes regarding authorization (if applicable)*

The certificate of findings in accordance with Law no. 359/2004*

Copy of the company's Articles of Incorporation*

Copy of the Sale-Purchase Agreement of the space, the lease agreement, as applicable, and if it is property, Land Register Extract*

The standard declaration on own responsibility regarding the fulfillment of the operating conditions provided by the specific legislation in the fields of: health, veterinary health, environmental protection, labor protection, and fire defense, as well as the specific regulations for consumer protection for the activity carried out according to the quality standards of the products and services placed on the market*

(property deed, lease, partnership or concession contract etc.), space blueprint (showing the realization of functional characteristics; service space, preparation-delivery area, storage area, utility annexes)*

Copy of the utilities-sanitation services contract and copy of the latest payment invoice*

In the case of carrying out construction modifications in the activity space or in the case of new constructions for which a building permit has been obtained and for which the reception and the reception report upon completion of the works have been carried out*

The fee for issuing the Agreement is paid into account RO10TREZ27421360250XXXXX opened at the Moreni Treasury and is calculated as follows: 100 lei for areas < 100 sqm, 200 lei for 101-200 sqm, 300 lei for 201-500 sqm, 4000 lei for areas > 500 sqm.*
