In the case of incorrect completion due to the fault of the educational institution that issued the original diploma or in the case of presenting the original diploma deteriorated so that it can no longer produce the effects for which it was issued, based on a written request by the applicant, the educational institution that issued the document will issue a duplicate, prepared based on documents found in the school’s archive that show the applicant’s academic status.
In the case of loss, destruction, or theft of the original diploma, the request for issuing the duplicate is accompanied by the following documents, which are archived with a retention period of 100 years at the school inspectorate, while certified copies "in accordance with the original" are kept at the educational institution:
a) a notarized/consular statement given by the holder of the diploma, in which all necessary elements for identification are included, the circumstances in which the original document was lost, as well as other details to confirm the disappearance of the respective diploma. The following mention must be obligatorily made: "the original diploma was not left as collateral/guarantee at a public or private institution, in the country or abroad, and was not knowingly transferred to another individual or legal entity";
b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;
c) notarized copy of birth/marriage certificate;
d) two color photographs, size 3 x 4 cm;
e) written approval of the school inspectorate’s management board for issuing the duplicate, following the documents submitted by the educational institution.
