The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is obliged to pay up to date the membership fee owed to the college from which they wish to leave. The transfer note will be handed over, with a signature, to the applicant. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint against the pharmacist or a disciplinary investigation has been registered at the college from which the pharmacist is transferring, the complaint or the disciplinary file will be sent to the college to which the pharmacist is transferring.
