The request of the beneficiary or the owner, as applicable, specifically mentioning the area, the investment to be made, Land Registry number, Cadastral number and/or topographic number, signature/stamp (legal persons), accompanied by a copy of the identity document;*

In case of representation, notarial power of attorney, respectively delegation, the decision of the general meeting of associations, the decision of the sole associate, the decision of the representative of the associative form, as applicable, in original and copy, as well as a copy of the ID card for the authorized natural person or a copy of the passport for the authorized natural person with domicile abroad;*

Copy of the ownership document (for example; ownership title, sale-purchase contract, donation contract) or another valid land possession document (for example; concession contract, lease, rental, superficies, etc.) with notarized authentication of the lease; (with the exception of requests for works provided in art.7 paragraph 14 of Law no. 50/1991 regarding the authorization of construction works, republished);*

The owner's agreement for carrying out the investment objective on the studied site, in case the land is owned by a person other than the owner; the administrator's agreement for carrying out the investment objective on the studied site, in case the land belongs to the public/private domain of the state - in original and copy;*

Copy of the urban planning certificate (valid), accompanied by the zoning plan and the site plan, as applicable;*

Payment can be made by the following methods: - cash at the central unit - bank transfer at the banking unit - treasury ANIF VASLUI ACCOUNT RO58TREZ656501701X006407*

The land register excerpt for information, accompanied by the cadastral plan excerpt, issued by the office of cadastre and real estate publicity no more than 30 days before the date of submission of the documentation, as the case may be (except in cases where the construction authorization is requested for the realization and rehabilitation of land improvement infrastructure in accordance with the provisions of art. 33, para. 2, letter a of law 138/2004); (except for requests for works provided in art. 7, para. 14 of law no. 50/1991 regarding the authorization of construction works, republished); (except for public utility investment works)*

Copy of the ANIF approval for drafting the PUZ (accompanied by the Local Council Decision approving the PUZ), for verifying compliance with the conditions imposed when issuing the approval (if applicable);*

Where the situation requires it (when in the Urbanism Certificate, according to Law no. 133/2012, there is a mention of agricultural intravilan, or for a PUD request when there are IF works that may be affected), the beneficiary's file must include a Project with the technical solution for protecting the IF works, signed and stamped by a person authorized by ANIF and also, according to art. 47^1 of Law no. 350/2001, republished, the file must include*

Certificate of registration and certificate of status from the Trade Register or the document based on which it operates, in copy, in the case of the beneficiary legal entity;*
