The application must be accompanied by the documents provided in art. 5 of the Accreditation Criteria for providers of specialized services for stimulating employment, approved by Government Decision no. 277/2002, as amended and supplemented.*

The document is presented in a copy certified by the supplier and may be, as appropriate: the registration certificate at the ONRC, the authorization for carrying out independent activities, the certificate of registration in the Register of Associations and Foundations.*

In the situation where there is an obligation to use the CAEN code, the object of activity is activities of employment placement agencies CAEN code 7810. In the situation of other entities established according to the law, for which there is no obligation to use the CAEN code, the object of activity can be, for example: information and professional counseling services. It is submitted in the situation where the document proving the registration of the provider does not specify the object of activity. For providers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the state of origin issued for an equivalent purpose or from which it is clearly evident that the requirements of the document are met is accepted. Documents issued in other EU and EEA member states are submitted as a copy certified by the provider, accompanied by an uncertified translation in Romanian.*

Attest eligibility. Supporting documents or a declaration on the provider's own responsibility, showing that they have not committed crimes in Romania or, as the case may be, in the country of origin in connection with the employment services they provide. For providers from the European Union and from the European Economic Area subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or which clearly shows that the requirements of the document are met is accepted. Documents issued in other EU and EEA states are submitted in a copy certified by the provider, accompanied by an uncertified translation into Romanian.*

Attest eligibility. Supporting documents or a statement on the supplier's own responsibility attesting that the supplier is not in Romania or, where applicable, in the country of origin, in a situation of insolvency, in enforcement proceedings, judicial reorganization, bankruptcy, dissolution, liquidation, or special administration, does not have the activities by which it provides employment services suspended or restrictions upon them, there are no final court rulings of conviction related to violations of legislation regarding the provision of employment services, or is not in other similar situations regulated by law. For suppliers from the EU and EEA who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or which clearly shows that the requirements are met is accepted, the document being submitted in a certified copy by the supplier accompanied by an uncertified translation in Romanian.*

Attests technical and managerial capacity. Submitted in a certified copy by the supplier. For suppliers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or which clearly shows that the requirements of the document are met, is accepted. Documents issued in other EU and EEA member states are submitted in a certified copy by the supplier, accompanied by an uncertified translation into Romanian.*

Attests technical and managerial capacity. Declaration on the provider's own responsibility regarding the existence of the material base provided in art.3 letter B letter a)-c) from the accreditation criteria approved by Government Decision no.277/2002, with subsequent amendments and completions, including specific equipment, technical equipment, and other fixed assets that they undertake to use for carrying out the service for which accreditation is requested. For providers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or from which it is clearly evident that the requirements of the document are met is accepted. Documents issued in other EU and EEA member states are submitted in a copy certified by the provider, accompanied by an uncertified translation into Romanian.*

Attests technical and managerial capacity. Declaration on the supplier's own responsibility containing information regarding the description of the service provided, respectively the method of organization and established objectives, target groups, provision of specialized personnel. For suppliers from the European Union and the European Economic Area who are subject to the accreditation obligation, any document from the country of origin issued for an equivalent purpose or which clearly shows that the requirements of the document are met is accepted. Documents issued in other EU and EEA member states shall be submitted in a copy certified by the supplier, accompanied by an uncertified translation into Romanian.*

It certifies technical and managerial capacity. It certifies staffing, as provided in art. 3 letter C of the accreditation criteria approved by Government Decision no. 277/2002, with subsequent amendments and completions. Experience in the field of the service for which accreditation is requested is certified by: European model curriculum vitae. For providers from the European Union and the European Economic Area who are subject to the accreditation requirement, any document from the state of origin issued for an equivalent purpose or clearly showing that the requirements of the document are met, is accepted. Documents issued in other EU and EEA member states are submitted in a copy certified by the provider, accompanied by an uncertified translation into Romanian.*

Document attesting payment of the accreditation fee, either directly at the AJOFM/AMOFM Bucharest headquarters or via bank transfer to the AJOFM/AMOFM Bucharest account.*
