For obtaining information and submitting documentation, the applicant will address the Citizen Information Center, room 15 within the Rm. Vâlcea City Hall.

- Copy of applicant's ID card as well as those of household members who will be registered on the certificate and booklet
- Proof of payment of fees according to HCL 259/29.12.2014 Regarding the establishment and approval of the level of taxes, fees and other amounts due to the local budget for the year 2015, supplemented by HCL 74/29.04.2015.
