In the case of an incorrect completion due to the fault of the educational institution that issued the original diploma or in the case of presenting the original diploma damaged to such an extent that it can no longer serve its intended purpose, based on a written request from the applicant, the educational institution that issued the respective document shall issue a duplicate, drawn up on the basis of documents held in the archive of the educational institution that prove the applicant's academic record.
In case of loss, destruction, or theft of the original diploma, the request for issuing a duplicate must be accompanied by the following documents, which are archived for a period of 100 years at the County School Inspectorate, while the certified true copies are kept at the educational institution:
a) the notarial/consular statement made by the holder of the act, which must include all identification elements, the circumstances under which the original was lost, as well as other clarifications confirming the disappearance of the respective diploma. The following mention is mandatory: "the original diploma was not used as collateral/guarantee at a state or private institution, in the country or abroad, and it was not knowingly transferred to another natural or legal person";
b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;
c) notarized copy of the birth/marriage certificate;
d) two color photographs, size 3 x 4 cm;
e) written approval of the board of directors of the school inspectorate for issuing the duplicate, following the documents submitted by the educational institution.
