The standard application is submitted or sent by mail with acknowledgment of receipt from OE together with the authorization file. The application must include: the applicant's contact details (name, surname, postal address, email address), the types of courses for which authorization is requested, the lecturers used in the professional training processes, the transport manager’s information, etc.*

The manager of the professional training center declares that, being aware of the provisions of the Criminal Code regarding false statements, certifies that the information provided in the application is true and that the documents submitted in scanned form on the electronic support attached to the authorization application are in accordance with the originals.*

a) the document proving the fulfillment of the condition provided in art. 5 of the Norms approved by ORDER MT no. 1.212/2015; b) the list including the minimum technical-didactic facilities, provided in art. 7 para. (2) letter c) and, as the case may be, d) of the Norms approved by ORDER MT no. 1.212/2015; c) for the types of courses for which the curriculum is not provided in a normative act, the training curriculum appropriate to the types of courses for which authorization is requested, in accordance with the legislation in the field, in which the topics and duration of the course structured by hours will be highlighted; if the legislation in the field does not provide otherwise, the training courses will be scheduled so that each training day includes no more than 8 hours of 60 minutes each, including breaks; d) for the manager of the training and professional improvement center: (i) the appointment decision stating that he/she permanently and effectively leads the activity of the training and professional improvement center and is mandated to represent the center in the relationship with the authorities; (ii) higher education diploma; (iii) documents proving that he/she has the necessary seniority in the field of road transport; (iv) the employment contract concluded according to the law; and (v) the criminal record certificate and the history of sanctions regarding the traffic rules on public roads or, in the case of citizens of other member states of the European Union or the European Economic Area, the notarized declaration on their own responsibility attesting the fulfillment of the condition provided in art. 7 para. (1) letter c) of the Norms approved by ORDER MT no. 1.212/2015; e) the list containing the road transport lecturers who will teach the courses for which authorization is requested, indicating the type of course for each lecturer; f) for the road transport lecturers who will teach courses: (i) proof of certification as a road transport lecturer in the specialty of the type of course they will teach; (ii) the employment contract concluded according to the law; and (iii) the criminal record certificate and the history of sanctions regarding the traffic rules on public roads or, in the case of citizens of other member states of the European Union or the European Economic Area, the notarized declaration on their own responsibility attesting the fulfillment of the condition provided in art. 7 para. (1) letter c) of the Norms approved by ORDER MT no. 1.212/2015; g) the course support for each type of course for which authorization is requested, drawn up so that its content takes into account the requirements and regulations in force in the road transport field, valid on the date of application for the authorization of the training and professional improvement center. The course support must be drawn up in accordance with the training programs including those provided at letter c), and the content must be adapted to the learning needs of the candidates and the objectives of the course; h) in the case of requesting authorization to carry out types of courses that require practical training/examination, the list containing the vehicles used in the professional training activity, as well as the documents related to each vehicle, namely: (i) the registration certificate and the identity card of each owned vehicle; and (ii) the document proving the ownership of each vehicle.*

Additionally, for authorization for training and improvement courses for safety advisers and/or drivers for the road transport of radioactive materials belonging to class 7 of the A.D.R., course approvals are submitted in the authorization file.*
