the applicant’s declaration on their own responsibility regarding the fact that they are not in any of the following situations: - suffers from chronic diseases, certified by the family doctor, which could endanger the child's health; - ; - suffers from mental illnesses, is addicted to alcohol, drugs, or other psychotropic substances; - has been deprived of parental rights by a final court decision or has had parental rights prohibited as a complementary punishment; - has been convicted by a final court decision, for intentionally committing crimes against persons, crimes that affect relationships regarding social cohabitation, or those that hinder the administration of justice; - has a child who benefits or has benefited from a special protection measure for reasons attributable to the parents, such as abuse, neglect, or any form of violence against the child, begging, etc.; - has submitted a request for the issuance of the adopter/adoptive family certificate, except in cases where the person or family is part of the extended family; - actually lives or lives and manages the household together with the parents of the child/children for whom the placement measure is to be applied, except in cases where the mother is a minor, the parents are persons with disabilities or suffer from mental illnesses; - has been a maternal assistant and had the certificate withdrawn for reasons attributable to them; - temporarily exercises parental authority over the child during the absence of the parents, according to the provisions of art. 105, paragraph 3 of Law no. 272/2004, republished, with subsequent amendments and completions (The Court shall order the temporary delegation of parental authority over the child, during the absence of the parents, but for not more than one year, to the person designated by the guardianship court, in accordance with the provisions of the law).*
