Recent medical documents, recommendations from the specialist doctor, accompanied by documents issued by the competent authorities in the field of social health insurance, indicating that the medical investigations, surgical interventions, medication treatment, medical devices, or necessary therapies recommended by the specialist doctor are not covered by the social health insurance system, or, if they are partially covered, the amount must be mentioned; a copy of the document on the basis of which approval for treatment abroad was granted, supporting documents regarding the necessary expenses to be made, issued by the medical service provider who will carry out the surgical intervention/post-operative treatment, translated by an authorized translator, documents issued by the competent authorities in the field of social health insurance, showing that these are not covered by the social health insurance system.
