Copy of the beneficiary's identity document, in the case of individuals, or a copy of the single registration certificate, in the case of legal entities or registered individuals.*

Technical presentation memorandum of the requested work for original approval, which should include: the existing situation – delimitation of the existing built-up area of the locality/commune, indicating exactly the county road or roads crossing the locality, the kilometer positions on the county road, the existing buildable limits; the territory included between the built-up area limit and the administrative territorial limit, with kilometer positions. The projected situation: - reservation of land for future road developments (with at least the width of one traffic lane in alignment – or according to the viewpoints of the designers of county road modernization/widening works, and in compliance with visibility conditions on the road). - construction of roads parallel to the county road, meant to take over the traffic generated by objectives located in the area of the county road and which should exit onto the county road in intersections properly arranged according to traffic volumes. - The extension of the built-up area along the county road will be done only if collector roads parallel to the county roads are arranged, which will exit into intersections arranged according to current standards. These collector roads shall be arranged along the entire length of the built-up area extension, on both the left and right sides. *In case there is not enough land for the execution of the respective collector roads, the city hall may extend the localities’ built-up area laterally, and, for the connection to the county roads, it can develop a catenary road network which will exit onto these, in properly arranged intersections. - reservation of necessary spaces for placing utilities, public transport stations, parking lots, etc., outside the safety zone of the county road. - kilometer positions of the entrance and exit town signboards, specifying that within these signposts the built-up area traffic rules apply (especially speed limit). - kilometer positions of the intersections with other roads (national, county, communal) and specifying the proposal for their modernization or not. - specifying the proposed built-up area to be included in the built-up area (beginning and end kilometer positions), correlated with the existing one. - the safety and protection zone of the county road must be specified, in built-up areas, in existing built-up areas, in the area of the built-up area extension and in non-built-up areas, according to Government Ordinance 43/1997, the positioning distance of fences located on both sides of the county road. - For the projecting, execution and interventions on roads, consideration will be given to their functional categories, car traffic, traffic safety, technical regulations, economic, social, and defense factors, rational land use, conservation and environmental protection, and urban and territorial planning approved according to the law, as well as current technical norms for adapting these to pedestrian, cyclist, disabled, and elderly requirements. - According to Government Ordinance 43/1997, art. 28 – the arrangement of level intersections between two or more roads shall ensure priority for the traffic running on the higher category road.*

Territorial integration plan on which the location of the land will be marked.*

Situation plan at a convenient scale, in original, which should include: - the existing route of the county roads passing through the locality (highlighted), - the kilometer positions on the county road of the following boundaries: administrative, existing built-up area, proposed built-up area, existing/proposed collector roads. - the kilometer positions of intersections with other classified roads (national, county, communal), - the kilometer positions of intersections proposed to be modernized and of other elements related to traffic (bypass roads, collector roads, etc.).*

Request signed by the beneficiary in which the following must be specified: full name, full address, phone/fax number, e-mail.*

Cross-sections of the county road (from fence/construction to fence/construction) for: - Existing inner city area. - Proposed inner city area. - Outside the city limits. The cross-sections will be made on characteristic sectors, separately for the current situation of the county road area and separately for the proposed situation of the road area. They will include the county road and all its structural elements (carriageway, shoulder, ditch, embankment, sidewalk, bicycle lanes, collector roads, utilities area, etc.), as well as road zones (protection zone, safety zone, construction limit). For the areas of inner city expansion, spaces will be reserved for the modernization of the county road, collector roads, sidewalks, bicycle lanes, public transport stations, utilities area, etc.*
