The document is valid for 3 months from the date of issuance and must be submitted within its validity period. In the situation where, for access and exercise of the respective profession, the member state of origin or provenance does not issue this document to its citizens, the applicant shall submit an attestation equivalent to the health certificate, issued by that member state*

evidence issued by the member state of origin or provenance regarding honorable and moral character or the absence of any situation that suspends or prohibits the exercise of the profession in case of serious professional misconduct, namely a certificate confirming the nonexistence of temporary or definitive suspensions from practicing the profession or any criminal convictions. The document is valid for 3 months from the date of issue and must be submitted within the validity period. If the member state of origin or provenance does not issue this document to its citizens, the applicant shall submit a certificate issued on the basis of a sworn statement or a solemn declaration of the applicant, by the competent judicial or administrative authority or, as the case may be, by the notary or professional organization authorized for this purpose by that state.*
