STANDARD APPLICATION for the issuance of the operating permit in which the identification elements of the merchant will be entered: merchant name, registered office, unique registration code (CUI), registration number at the Trade Register Office, representative, address of the work point for which the operating permit is requested, activities to be carried out at this work point, proposed operating schedule. This standard application is accompanied by documentation that includes documents established correlatively with the activity to be carried out at the work point, in compliance with the relevant legislative provisions.*

The certificate of findings regarding the activities carried out at the working point that is the subject of the request, issued by the Trade Registry Office, contains strict information regarding the working point and lists the activities that are actually carried out at the working point. In the situation where this certificate of findings includes activities that are not actually carried out at the working point and for which an operating permit is not requested, the merchant's administrator will present a declaration made on their own responsibility regarding the activities carried out at the working point.*

The updated land register, not older than 90 days, of the property where the commercial activity or service provision takes place, including the entry of the corresponding function. The function registered in the land register shall be closely correlated with the provisions of the approved urban planning regulations, respecting the classification of permitted uses by categories of functions, according to the local urbanism regulations in force. In the event that, in the land register, the space in question is designated as a residence or has another purpose, but with a function different from the one for which the operating permit is requested, the urbanism certificate and the approvals/agreements mentioned therein / building permit, as the case may be, issued by the Urbanism Directorate for the change of use/function shall be presented, based on the provisions of the approved urban planning regulations, respecting the classification of permitted uses by categories of functions. In the case where the commercial/service activity is to be carried out in a temporary and removable construction, for which there is a building permit, such as a kiosk, booth, or stand or other similar structures (for commercial activity), located on private property (except those placed on the public or private domain of the municipality), the updated land register of the land on which it is located, not older than 90 days, shall be presented, if it is not possible to present a land register with the registration of this construction. An extract from the Land Register will NOT be presented in the case of spaces with another destination that are the subject of lease contracts with the local public administration authority.*

Traders who do not own the premises of the establishment to be authorized shall, in addition to the documents mentioned in the previous point, also submit to the file any document/contract regulating the lease in their favor. If this lease agreement is not for an indefinite period, it shall be considered that the authorization request is made for a limited period, within the validity of the lease.*

The written agreement of the co-owners - co-holders (where applicable) shall be given with the specification of the identification data of the signatories, namely the identity document and personal identification number*

The site plan showing the location of the building in which the commercial or service activity is carried out: site plans taken from the internet are also valid.*

A photograph of the facade of the sales structure will be presented, showing the access to it.*

The endorsed plan for unchanged status annexed to the building permit related to the sales structure space or the survey of the respective space will be presented.*

It is a standard form that includes information regarding the sales structure.*

The authorization regarding fire safety or denial issued by the competent authority Inspectorate for Emergency Situations "Avram Iancu" of Cluj County.*

The document issued by the Sanitary-Veterinary Directorate of Cluj County for the activities included in Annex 6 to HCL 851/2017 (see normative acts)*

The certificate of completion of hygiene courses or the diploma of graduation from a high school with an economic profile, for employed personnel, will be presented.*

The sanitation contract concluded with the sanitation company serving the respective area will be presented.*

One of these documents will be presented for the sales structure to be authorized.*

A copy of the building permit/subscription for occupying public domain with access roads, valid at the time, will be presented in the situation where access to the sales structure subject to authorization is made on distinct arrangements built on public domain, other than those corresponding to public alleys. In case the sales structure does not have an access road, a declaration made on one’s own responsibility by the trader will be submitted.*
