In the case of incorrect completion due to the fault of the educational institution issuing the original diploma or in the case of presenting the original diploma deteriorated so that it can no longer produce the effects for which it was issued, based on a written request from the applicant, the educational institution that issued the respective document issues a duplicate, drawn up based on the documents found in the archive of the educational institution from which the applicant's school situation results.

In the case of loss, destruction, or theft of the original diploma, the request for issuing a duplicate is accompanied by the following documents, which are archived for a retention period of 100 years at the county school inspectorate, and certified true copies remain at the educational institution:

a) notarial/consular statement, given by the holder of the document, which includes all elements necessary for identification, the circumstances in which the original was lost, as well as other clarifications to confirm the disappearance of the respective diploma. The following statement will be made compulsorily: "the original diploma has not been used as collateral/guarantee at any public or private institution, in the country or abroad, and has not been knowingly transferred to another natural or legal person";

b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;

c) notarized copy of the birth/marriage certificate;

d) two color photos, size 3 x 4 cm;

e) written approval from the board of directors of the county school inspectorate for issuing the duplicate, following the documents submitted by the educational institution.
