Persons who do not receive any other social benefit (e.g.: family support allowance or social aid) fill in Annex no. 1 to Government Decision-Annex no. 1*

Persons who benefit from family support allowance will complete Annex 2 to Government Decision - Annex 1a*

- identity document of the applicant and of all family members over 14 years old (B.I., C.I., C.I.P.); - birth certificate of children under 14 years old; - marriage certificate – if applicable; - divorce decree/notarial act – if applicable;*

- sale-purchase contract; - sale-purchase contract with maintenance clause or with lifelong habitation clause; - rental contract; - loan for use contract; - heir certificate/succession act; - other documents, as applicable.*

- pension slips (for all types of pensions) / certificate from the Pension House from the month prior to the submission of the application; - unemployment benefit payment slip and/or the monthly unemployment supplement income; - state allowance slip, placement or other allowances, from the month prior to the submission of the application; - disability allowance slip/complementary budget or any other allowances granted according to the law – from the month prior to the submission of the application; - income from leasing, bank interest, sale of goods – if applicable; - affidavit from the holders/members who do not earn income – if applicable. The copy will be handwritten certified as conforming to the original. - employment certificate for all family members, from which the net income obtained in the month prior to the submission of the application, the value of meal vouchers or mention that no vouchers, bonuses, food allowances, etc. are received – in original.*

- persons who own cars - copy of the car registration certificate (showing the year of manufacture of the car); -bank account statement The copy will be certified in handwriting for conformity with the original.*
