This procedure is addressed to the members of the College of Dental Physicians Salaj
The current professional certificate is the document that contains relevant personal data regarding honorability and morality, including the competence with which dentists exercise their profession. The current professional certificate is issued, upon request, by the Romanian College of Dental Physicians, through the territorial colleges, for the purpose of guaranteeing the quality of the services provided and the safety of patients in relation with it, based on the provisions of art. 512 para. (1) letter d) of Law no. 95/2006 regarding the reform in the health sector, republished, with subsequent amendments and completions, of art. 1 of the methodological rules approved by Government Decision no. 1464/2006 regarding the preparation, issuance and use of proof of professional honorability and morality of doctors, dentists and pharmacists, as well as the Decision of the National Council of CMSR no. 16/2CN/2019 regarding the preparation and issuance of the current professional certificate. The validity period of the current professional certificate is 3 months from the date of issuance.
Certificate issuance fee 500 lei. The fee can be paid at the headquarters of the College of Dental Physicians Salaj or in account RO42BTRLRONCRT0382431101 opened at Banca Transilvania, Zalau branch, beneficiary College of Dental Physicians Salaj, CUI: 18198734. In the payment details, the name of the dentist and the reason for payment will be mentioned (e.g.: Popescu Ioana certificate issuance fee for the current professional certificate).
