Resumption of membership in the College of Pharmacists*
The reinstatement of membership is based on a request, which the requesting pharmacist submits to the Prahova College headquarters, through which they request the reinstatement of membership starting from a specific date. In support of the request, the documents listed below are attached.
Transfer pharmacists to another territorial college*
The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is obliged to pay up to date the membership fee owed to the college from which they wish to leave. The transfer note will be handed over, with a signature, to the applicant. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint against the pharmacist or a disciplinary investigation has been registered at the college from which the pharmacist is transferring, the complaint or the disciplinary file will be sent to the college to which the pharmacist is transferring.

Current Professional Certificate Request*
The methodology for preparing and issuing the Current Professional Certificate was established by the Decision of the National Council of the C.F.R. no. 1/2007, decision published in the Official Gazette of Romania Part I no. 324 of 15.05.2007.
Below are the main provisions of Decision no. 1/2007, as follows:
- The Current Professional Certificate will be prepared by the Romanian College of Pharmacists and issued by the territorial college where the applicant is registered;
- The validity of the Current Professional Certificate is 3 months from the date of issue;
- Within a maximum of three days from receiving the application for the issuance of the Current Professional Certificate, the territorial college will send the standard request to the Romanian College of Pharmacists for the preparation and issuance of the document;
- Within a maximum of 30 days from receiving the request, the C.F.R. will prepare the certificate and forward it to the territorial college;
- The Current Professional Certificate will be issued by the Romanian College of Pharmacists, which will prepare and forward it to the territorial college;
- The territorial college will maintain a Register of evidence – with the identification data of Current Professional Certificates issued to pharmacists in the respective county – and will hand them to applicants with signature upon receipt.
Duplicate certificate of membership of the College of Pharmacists of Romania*
In order to proceed with issuing a duplicate of the Certificate of Membership of the College of Pharmacists of Romania, the applicant must submit the complete documentation at the headquarters of the Prahova College of Pharmacists, according to the provisions of Decision no. 4/2004 of the National Council of the College of Pharmacists of Romania and the documents presented below.

Registration in the College of Pharmacists of Romania*
The membership certificate is granted to pharmacists who, upon explicit request, meet the following conditions: they hold an official pharmacy qualification, are not in any cases of unworthiness or incompatibility provided by law, are medically fit to practice as a pharmacist, have not interrupted their professional practice for more than 5 years or have obtained their bachelor's degree in the last 5 years, have paid the registration fee (the fee is to be paid into the account of the territorial college RO91BREL0002001133430100 in the amount of 208 lei for persons who registered within 1 YEAR of graduation and 235 lei for other persons).
