A copy of the valid authorization card/certificate for authorized electrician, issued according to the law by the competent national regulatory authority, hand-certified by the applicant for conformity with the original for subdomains 8.1 Electrical installations and 9.1 Electrical networks. A copy of the valid authorization card/certificate for authorized natural gas installer, issued according to the law by the competent national regulatory authority, hand-certified by the applicant for conformity with the original, for subdomains 8.3 - Natural gas installations and 9.4 - Natural gas networks. Mandatory documents for applicants wishing to obtain authorization in subdomains 8.1/9.1/8.3/9.4. Proof of holding these types of authorizations for at least 5 years must also be submitted.*

Judicial record certificate, valid at the date of application registration, showing that the applicant is not listed in the judicial record or Consent for obtaining an extract from the judicial record - Site supervisors by I.S.C. - according to the attached model.*
