Request for certification – Annex 7 of the Regulation*

Certificate issued by the Trade Registry Office, in copy, reflecting the updated situation of the applicant at the date of the attestation request, regarding their status, identification data, and communication channels, showing that the applicant has registered as a main or secondary activity activities of the type subject to attestation; in the case of individuals, documents proving that the applicant meets the legal conditions for the organization and conduct of economic activities by individuals, being registered in Romania as an authorized natural person, individual enterprise or family enterprise.*

List including the technical-material equipment, which demonstrates the applicant's capability to carry out activities subject to certification – Annex 4 and Annex 8 of the Regulation.*

Structure of employed/contracted personnel, allocated to activities subject to certification – Annex 5 and Annex 9 from the Regulation.*

Extract from the Electronic Register of Employees (REVISAL), dated and signed by the legal representative of the applicant for certification, showing the status of employees of the persons in the personnel structure or, as appropriate, certified copies of the service contracts concluded with them.*

Statement on own responsibility of the legal representative – Annex 2 from the Regulation.*

The Quality Management System Certificate issued by one of the companies accredited by RENAR or by another similar international body, stating that the applicant carries out the activities subject to certification under the conditions of the ISO 9001 standard.*
