Certification request – Annex 7 from the Regulation*

Certificate issued by the Trade Registry Office, in copy, reflecting the updated situation of the applicant as of the date of the attestation request, regarding their status, identification data, and means of communication, and showing that the applicant has registered as main or secondary business activities those activities subject to attestation; in the case of natural persons, documents proving that the applicant meets the legal requirements for the organization and conduct of certain economic activities by natural persons, being registered in Romania as an authorized natural person, individual enterprise or family enterprise.*

The list comprising the technical and material equipment, which certifies the applicant's capability to carry out activities subject to certification – Annex 4 and Annex 8 of the Regulation.*
