In case of incorrect completion due to the fault of the educational institution issuing the original diploma, or in case the original diploma is presented in such a deteriorated condition that it can no longer produce the effects for which it was issued, based on a written request from the applicant, the educational institution that issued the document shall issue a duplicate, drafted based on the documents in the archive of the educational institution that demonstrate the applicant's academic records.
In case of loss, destruction, or theft of the original diploma, the request for the issuance of the duplicate must be accompanied by the following documents, which are to be archived for 100 years at the school inspectorate, while the certified true copies are kept at the educational institution:
a) notarial/consular statement made by the holder of the document, containing all the necessary identifying information, the circumstances in which the original document was lost, as well as other details to confirm the disappearance of the respective diploma. The following mention is mandatory: "the original diploma was not left as collateral/guarantee at a public or private institution, in the country or abroad, and was not deliberately transferred to another individual or legal entity";
b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;
c) notarized copy of the birth/marriage certificate;
d) two color photographs, size 3 x 4 cm;
e) written approval of the school inspectorate's board of administration for the issuance of the duplicate, following the submission of documents by the educational institution.
