In case of incorrect completion due to the fault of the educational unit issuing the original diploma or in case of presenting the original diploma deteriorated in such a way that it can no longer produce the effects for which it was issued, based on a written request from the applicant, the educational unit that issued the respective document will issue a duplicate, prepared based on documents found in the archive of the educational unit which show the applicant's academic record.
In case of loss, destruction, or theft of the original diploma, the request for issuing a duplicate must be accompanied by the following documents, which will be archived with a retention period of 100 years at the school inspectorate, while certified copies "in accordance with the original" will be kept at the educational unit:
a) a notarial/consular declaration, given by the holder of the document, which includes all necessary identification elements, the circumstances in which the original diploma was lost, as well as other comments confirming the disappearance of the respective diploma. The mandatory annotation will be made: "the original diploma was not left as collateral/guarantee at a state or private institution, either domestically or abroad, and was not knowingly transferred to another individual or legal entity";
b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;
c) notarized copy of the birth/marriage certificate;
d) two color photographs, size 3 x 4 cm;
e) written approval from the board of directors of the school inspectorate for the issuance of the duplicate, after submission of the documents by the educational unit.
