In case of incorrect completion due to the fault of the educational unit issuing the original diploma or in case the original diploma is presented damaged so that it can no longer produce the intended effects, based on a written request from the applicant, the educational unit that issued the respective diploma will issue a duplicate of it, drawn up on the basis of documents in the archive of the educational unit from which the applicant’s academic record results.
In case of loss, destruction or theft of the original diploma, the request for a duplicate is accompanied by the following documents, which are archived with a retention period of 100 years at the school inspectorate, while certified copies "according to the original" are kept at the educational unit:
a) notarial/consular statement given by the holder of the diploma, which includes all necessary identification details, the circumstances under which the original diploma was lost, as well as other clarifications confirming the disappearance of the respective diploma. The following mention is mandatory: "the original diploma was not pledged/used as collateral at any state or private institution, from Romania or abroad, and was not knowingly transferred to another individual or legal entity";
b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;
c) notarized copy of the birth/marriage certificate;
d) two color photos, size 3 x 4 cm;
e) written approval of the board of the school inspectorate for issuing the duplicate, following the documents submitted by the educational unit.
