In the case of incorrect completion due to the fault of the educational institution issuing the original educational document or in the case of presenting the original educational document damaged to such an extent that it can no longer produce the effects for which it was issued, based on a written request from the applicant, the educational institution that issued the document will issue a duplicate thereof, drawn up based on the documents found in the educational institution's archive that show the applicant's academic status.

In the case of loss, destruction, or theft of the original educational document, the request for issuing the duplicate must be accompanied by the following documents, which will be archived with a retention period of 100 years at the school inspectorate, and the certified "true copy of the original" copies at the educational institution:

a) the notarial/consular statement, given by the holder of the document, which includes all necessary identification elements, the circumstances under which the original document was lost, as well as other details confirming the disappearance of the respective educational document. The following mention will be made in a mandatory manner: "the original educational document has not been pledged/used as collateral at any state or private institution, in the country or abroad, and has not been deliberately transferred to another individual or legal entity";

b) proof of publication of the loss, destruction, or theft of the educational document in the Official Gazette of Romania, Part III;

c) notarized copy of the birth/marriage certificate;

d) two color photographs, size 3 x 4 cm;

e) written approval from the school inspectorate's board of administration for issuing the duplicate, following the submission of the required documents by the educational institution.
