In order to proceed with the issuance of a duplicate Member Certificate of the College of Pharmacists of Romania, the applicant must submit the complete documentation to the headquarters of the College of Pharmacists Bucharest, in accordance with the provisions of Decision no. 4/2004 of the National Council of the College of Pharmacists of Romania, the documents presented below.
