In order to proceed with the issuance of a duplicate Member Certificate of the Romanian College of Pharmacists, the applicant must submit the complete documentation at the headquarters of the Bucharest College of Pharmacists, in accordance with the provisions of Decision no. 4/2004 of the National Council of the Romanian College of Pharmacists, the documents presented below.
