The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is obliged to pay the dues owed to the college from which they wish to leave. The transfer note will be handed, with signature, to the requester. In the pharmacist record registry (https://portal.tm-c.eu/LstFarma/ColegFarm) the college to which the pharmacist has transferred will be noted. If a complaint has been registered or disciplinary investigation initiated against the pharmacist at the college from which they are transferring, the complaint or disciplinary file will be forwarded to the college to which the pharmacist is transferring.
