The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is obliged to pay in full any dues owed to the college they wish to leave. The transfer note will be handed over, with signature, to the applicant. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint against the pharmacist has been filed with the college from which they are transferring, or if a disciplinary investigation has been initiated, the complaint or disciplinary file will be forwarded to the college to which the pharmacist is transferring.
