Required documents for the file (in copy): – standard application form – identity documents of the applicants; – child’s birth certificate; – as the case may be, marriage certificate or family record book; – certificate of disability classification for the child, if applicable; – approved application from the unit management for entering maternity leave for child raising; – proof issued by the employer or the competent authorities regarding the income earned; – decision to suspend the individual employment contract for the period of 1 year, 2 years or 3 years for the case of a child with disabilities; – in the case of persons who simultaneously earn taxable income from several sources, child-raising leave and allowance are granted based on supporting documents proving the suspension of at least one of the activities; – the applicant’s tax sheet (if applicable); – if the applicant chooses to receive the allowance or the incentive in their personal account, a bank statement and IBAN account number from the bank are required. If a person is already receiving the child-raising allowance and gives birth to another child, in order to extend the leave, a new file must be prepared. If the person on child-raising leave chooses to apply for the incentive, a new file must be prepared and an application for granting the incentive must be submitted. Starting with March 1, 2012, at least one month of the total period of child-raising leave is allocated to one of the persons who has not requested this right. Therefore, regardless of the situation, the parent who is on child-raising leave or on the integration incentive, except for cases where the applicability of art. 11 of GEO no. 111/2010 is not mandatory, will lose one month of entitlement, regardless of whether it is for allowance or incentive, if the other parent does not access their own right to leave. The new regulation applies for children born after March 1, 2012, this being imposed under the principle that the responsibility for child care lies with both parents.*

Documents required for the file (in copy): – application completed and signed by the child's legal representative; – the original and copy of the child's birth certificate; – identity document of the legal representative; – documents proving the child's legal status in relation to the legal representative and any other supporting documents, as applicable; – if the applicant chooses to receive the state allowance in a personal account, a bank statement is required, IBAN account from the bank.*
