Required documents for the file (in copy): – standard application form – identity documents of the applicants; – child's birth certificate; – if applicable, marriage certificate or family booklet; – certificate of classification of the child in a degree of disability, if applicable; – the application approved by the management of the unit for entering child-rearing leave; – proof issued by the employer or the competent authorities regarding the income earned; – order of suspension of the individual employment contract for the period of 1 year, 2 years, or 3 years in the case of a child with a disability; – in the case of persons who simultaneously obtain taxable income from several sources, leave and allowance for child-rearing are granted based on supporting documents regarding the suspension of at least one of the activities; – the applicant's fiscal record (if applicable); – if the applicant opts to receive the allowance or incentive in a personal account, a bank statement, IBAN account from the bank is required. If a person is receiving child-rearing allowance and has given birth to another child, for the extension of the leave a new file must be prepared. If the person on child-rearing leave opts for payment of the incentive, a new file must be prepared and an application must be submitted for granting of the incentive. Starting from March 1, 2012, at least one month of the total period of child-rearing leave is allocated to the parent who has not requested this right. Therefore, regardless of the situation, the parent who is on child-rearing leave or insertion incentive, except for cases where the applicability of art. 11 of GEO no.111/2010 is not mandatory, will lose one month of entitlement, whether it is the allowance or the incentive, if the other parent does not access their own right to leave. The new regulation applies to children born after March 1, 2012, this being imposed in accordance with the principle that the responsibility for child care lies with both parents.*

Required documents for the file (in copy): – application form filled in and signed by the child's legal representative; – original and copy of the child's birth certificate; – identity document of the legal representative; – documents proving the legal status of the child in relation to the legal representative and any other supporting documents, if applicable; – if the applicant opts to receive the state allowance in a personal account, a bank statement with the IBAN account number is required.*
