the applicant's own responsibility statement regarding the fact that he/she is not in one of the following situations: - suffers from chronic diseases, certified by the family doctor, that could endanger the child's health; - ; - suffers from mental illnesses, is dependent on alcohol, drugs, or other psychotropic substances; - has been deprived of parental rights by a final court decision or has been prohibited from parental rights as a complementary penalty; - has been convicted by a final court decision for intentionally committing crimes against the person, crimes affecting social coexistence relationships, or those that affect the administration of justice; - has a child who benefits or has benefited from a special protection measure due to reasons attributable to the parents, such as abuse, neglect, or any form of violence exercised against the child, begging, etc.; - has submitted an application for the issuance of the adopter/adoptive family certificate, except in the case where the person or the family is part of the extended family; - actually lives or lives and manages the household together with the parents of the child/children for whom the placement measure is to be applied, except when the mother is a minor, the parents are persons with disabilities, or suffer from mental illnesses; - has been a maternal assistant and had the certificate withdrawn for reasons attributable to him/her; - temporarily exercises parental authority over the child, during the parents' absence, according to the provisions of art.105, paragraph 3 of Law no.272/2004, republished, with subsequent amendments and completions (The court will order the temporary delegation of parental authority over the child, during the parents' absence, but not more than one year, to the person designated by the guardianship court, in accordance with the legal provisions).*
