regarding the applicant’s status and their relationship with the child/children for whom the right is being requested, for one of the persons who has adopted the child/to whom the child has been entrusted for adoption/who has the child in foster care/emergency foster care, with the exception of the professional maternal assistant who may benefit from these rights only for their own children, as well as the person who has been appointed as guardian. In the case of P.F.A (Authorized Natural Person): - Certificate from the Health Insurance House (C.A.S) stating the period during which the maternity (pregnancy + confinement) allowance was paid, or a declaration on own responsibility that no maternity allowance was received - Certificates from the Public Finance Administration indicating the income earned in the last 2 fiscal years prior to the child’s birth or, if applicable, proof of the income calculated for the advance payment of tax - Tax assessment decisions from the Public Finance Administration for the last 2 fiscal years prior to the child's birth showing the start date of activity, - The Single Declaration, for the last 2 fiscal years prior to the date of the child's birth. - The decision to suspend or cease activity, declaration 070, from the Public Finance Administration; For students, university graduates, pupils, etc.: - CERTIFICATE ISSUED BY THE EDUCATIONAL INSTITUTION ATTESTING THAT THE DAY COURSES WERE ATTENDED WITHOUT INTERRUPTION UNTIL THE DATE OF THE APPLICATION IN THE YEAR PRIOR TO THE CHILD’S BIRTH, OR DIPLOMA OF GRADUATION ACCOMPANIED BY THE TRANSCRIPT OR CERTIFICATE ISSUED BY THE HIGHER EDUCATION INSTITUTION IF THE FACULTY WAS GRADUATED IN THE YEAR PRIOR TO THE CHILD’S BIRTH IF THE PERSON IS IN ONE OF THE ASSIMILATED PERIODS m) attend, without interruption, the day courses of pre-university education, including within the program "A second chance", or, as the case may be, university studies at bachelor's or master's level, as well as post-university education at master's level, organized according to the law, in the country or abroad, in a field recognized by the Ministry of National Education, except for interruption of courses due to medical reasons; n) have the status of doctoral student, under the conditions provided by the National Education Law no. 1/2011, with subsequent amendments and completions o) are in the period between finishing one form of pre-university education and starting, in the same calendar year, another form of pre-university education, day courses, organized according to the law, attended without interruption; p) are in the period between graduation from day courses of pre-university education, organized according to the law, and starting university education, day courses, in the same calendar year; q) are in the period between completing one form of university education, day courses, with or without bachelor's or diploma examination, and starting, in the same calendar year, another form of university education, day courses, organized according to the law, attended without interruption; r) are in the period between completing one form of university education, at the level of bachelor's or master's degree, as well as post-university education at the master's level, day courses, and starting, in the same calendar year, another form of university education at the level of bachelor's or master's studies, day courses, organized according to the law, attended without interruption; s) are in the period between completing one form of post-university education, day courses, and starting, in the same calendar year, another form of post-university education, day courses, organized according to the law, attended without interruption; t) are in the 60-day period from the completion of compulsory education or, as the case may be, from graduation of day courses of pre-university, university at bachelor's or master's level and post-university at the master's level, organized according to the law, with or without graduation exam, for the purpose of employment or, as the case may be, entering unemployment, calculated starting from the 1st day of the month following the completion of studies - SELF-DECLARATION THAT NO INCOME SUBJECT TO TAX IS EARNED If the father requests the child raising and care allowance, it is also necessary to provide a certificate from the mother's employer specifying the period in which the maternity allowance was paid, i.e. prenatal and postnatal leave, or a self-declaration that no maternity allowance was received.*
