The procedure describes the activities carried out for granting free urban public transport for people with severe disabilities, persons with major disabilities, their personal assistants or escorts according to Law no. 448 of 2006 regarding the protection and promotion of the rights of persons with disabilities, with subsequent amendments and completions.

Requests for the issuance of the free urban public transport card are submitted to the Social Assistance Directorate Sibiu during the period 01 - 20 of each month, following which, after checking the eligibility conditions, the cards are completed and handed to applicants in the last 2 working days of the month.

Application form for granting free urban transport by surface public transport The application shall be filled out by the person with severe or pronounced disabilities / the parents of the minor with severe or pronounced disabilities / their legal representative / personal assistant - If the application is filled out by the parent of a child with severe or pronounced disabilities or by the legal representative of a person with severe or pronounced disabilities, then in the "Name" section, the name and surname of the parent/legal representative shall be specified; in the "First Name" section, the name and surname of the person with severe or pronounced disabilities (child or adult) shall be entered; and in the "CNP" item, the personal numerical code of the person with severe or pronounced disabilities shall be written. - The completed application must be signed, scanned, and uploaded.*
