Attention beneficiaries:

According to the legislation in force, the admission of people with disabilities or elderly persons into residential centers or public medico-social assistance units is only carried out if their assistance, care, recovery, or protection cannot be provided at home or in day centers organized in the community.

To facilitate access to such a center, the Rm. Vâlcea Social Assistance Directorate, through its specialized service, provides the social investigation - grid type (a working tool in the form of a standard sheet, provided by specific legislation, structured on a medical component and a social component), a mandatory document in the file to be submitted to the institution under whose authority the care center operates.

Documents required for carrying out the social investigation – grid type

￭ Certificate of disability classification, endorsed by DGASPC (if applicable);

￭ Civil status documents (identity bulletin/identity card, birth certificate, marriage certificate, or spouse’s death certificate, as applicable) for the person with disabilities or the elderly person;

￭ Civil status documents (identity bulletin/identity card, birth certificate, marriage certificate, or spouse’s death certificate, as applicable) for the legal representative (if applicable);

￭ Supporting documents regarding the income received, for the person with disabilities and the legal representative (if applicable);

￭ Medical assessment grid, completed by the family doctor or the attending specialist (form provided through the care of the DAS inspector handling the case);

￭ Court order/Judgment instituting guardianship/trusteeship (in the case of persons with mental health problems);

￭ Other documents supporting the state of socio-medical need,

￭ Standard application for the issuance of the social investigation, drafted by the person with disabilities / elderly person;

￭ Standard application for the issuance of the social investigation, drafted by the legal representative (if applicable);

ü If the person is admitted to a public sector care facility and receives an attendant’s allowance, this financial right ceases. The person concerned, the relative, or their legal representative has the obligation to inform the specialized department within DAS, in order to avoid subsequent recovery of unduly received amounts.
