The generator/holder of waste can submit a request to obtain the TRANSPORT APPROVAL (Annex 1 of HG 1061/2008) based on the following documents: 1. request for approval of the transport of hazardous waste generated in larger quantities 2. forms for approval of hazardous waste transport - Annex 1 (6 original copies) 3. proof of payment for the approval procedure for internal transport of hazardous waste. - Additional information may be requested if necessary - approval of transport forms - issuance of Annex 1 forms in the 6 original copies
