Details

Case management department for children in family-type services – AMP ensures the protection of the child who requires the establishment of a protection measure that entails the upbringing, care, and education of the child by a professional foster carer, at their home. Professional foster carer (AMP)- a certified individual who, through the activity carried out at their home, ensures the upbringing, care and education necessary for the harmonious development of the children they receive in foster care or custody.

Individuals residing within the territorial area of Suceava county, graduates of vocational school or high school, interested in becoming a foster carer, must submit to the General Directorate of Social Assistance and Child Protection Suceava a request for assessment of their capacity, accompanied by the documents listed below.

Certificate- the document issued by the Child Protection Commission, in accordance with the provisions of Ordinance no. 27/2024.The certificate is issued for an undetermined period of time and is approved by CPC every two years.

The applicant submits to the DGASPC headquarters, Case Management Department for Children in Family-type Services, the standard form, to which he/she attaches the CV template and the documents listed in the standard request.*

At the request for evaluation, the applicant shall attach the following documents: a) curriculum vitae; b) copy of the identity card; c) copy of the identity card for the persons he/she lives with; d) copy of the birth certificate; e) copy of the birth certificate for the persons he/she lives with; f) copy of the marriage certificate or the divorce decision/divorce certificate, if applicable; g) copy of the diploma (to be specified); h) copy of the property title or any other document attesting the right of use of the dwelling; i) criminal record certificate; j) criminal record certificate for persons over 14 years old with whom he/she lives; k) behavioral integrity certificate; l) behavioral integrity certificates for the adult persons he/she lives with; m) income certificate; n) medical certificate issued by the family doctor showing that the applicant does not suffer from communicable diseases; o) medical certificate, for each adult person he/she lives with, issued by the family doctor showing that they do not suffer from communicable diseases; p) medical certificate issued by a specialist doctor showing that he/she does not suffer from mental illness / is not dependent on alcohol, drugs or other psychotropic substances; q) medical certificate, for each adult person he/she lives with, issued by a specialist doctor, showing that they do not suffer from mental illness / are not dependent on alcohol, drugs or other psychotropic substances; r) affidavit from which it appears that the applicant is not in any of the situations provided by law in which a person cannot become a foster carer; s) recommendation(s).*
