persons with residence abroad will present their passport and a certificate of their last domicile in the country, issued by the assigned SPCLEP, and proof regarding their legal status towards the Romanian state, in case they cannot prove Romanian citizenship - original and copy.*

of the person requesting the change of surname and/or first name, - certified copies.*

in case of changing the common family name used during the marriage, - original.*

in the case of changing the surname or first name of the minor child. This is not necessary if the other parent is placed under interdiction or is declared missing by court decision or is deprived of parental rights*

in the situation where the parents do not agree regarding the change of the minor children’s name, as well as in the case where the request to change the family name and/or the first name is made by the guardian for minors and persons placed under judicial interdiction,- copy*

Archive document search fee - 8 lei. Processing and analysis fees for the file in order to resolve the request for changing the surname and/or first name administratively - 400 lei or 700 lei - urgent processing - within 7 working days from the date the request is registered at DPJEP.*
