In the case of incorrect completion due to the fault of the educational institution issuing the original diploma or in the case of presenting the original diploma damaged such that it can no longer produce the effects for which it was issued, based on a written request from the applicant, the educational institution that issued the respective document issues a duplicate, made on the basis of documents found in the archive of the educational institution which reflect the academic status of the applicant.

In case of loss, destruction, or theft of the original diploma, the request to issue the duplicate must be accompanied by the following documents, which are archived for 100 years at the school inspectorate, and the certified copies "according to the original" at the educational institution:

a) a notarial/consular declaration, made by the holder of the document, which includes all elements necessary for identification, the circumstances in which the original document was lost, as well as other clarifications to confirm the disappearance of the respective diploma. The following statement must be obligatorily included: "the original diploma was not left as a pledge/guarantee at any state or private institution, in the country or abroad, and was not knowingly alienated to another individual or legal entity";

b) proof of publication of the loss, destruction, or theft of the diploma in the Official Gazette of Romania, Part III;

c) notarized copy of the birth/marriage certificate;

d) two color photos, size 3 x 4 cm;
