In the case of incorrect completion due to the fault of the educational institution issuing the original certificate or in case the original certificate is presented in such a deteriorated state that it can no longer serve its intended purpose, based on a written request from the applicant, the educational institution that issued the respective certificate issues a duplicate thereof, prepared based on the documents in the archive of the educational institution from which the applicant's academic record results.

In the case of loss, destruction, or theft of the original certificate, the request for issuance of the duplicate must be accompanied by the following documents, which are archived for 100 years at the County School Inspectorate, while copies certified as "true to the original" remain with the educational institution:

a) Notarial/consular statement, given by the holder of the certificate, containing all necessary identification details, the circumstances under which the original certificate was lost, as well as other clarifications to confirm the disappearance of the respective certificate. The following statement is mandatory: "the original certificate was not pledged/used as collateral at any public or private institution, in the country or abroad, and was not knowingly transferred to another individual or legal entity";

b) Proof of publication of the loss, destruction, or theft of the certificate in the Official Gazette of Romania, Part III;

c) Notarized copy of the birth/marriage certificate;

d) Two color photographs, size 3 x 4 cm;

e) Written approval of the board of administration of the County School Inspectorate for the issuance of the duplicate, following the documents submitted by the educational institution.
