The transfer note is issued by the college where the pharmacist is registered, based on a request. Prior to the issuance of the transfer note, the requesting pharmacist is obliged to pay up to date the membership fee owed to the college they wish to leave. The transfer note will be handed, under signature, to the applicant.
In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If, at the level of the college from which the pharmacist is transferring, a complaint has been registered against them or a disciplinary investigation has been initiated, the complaint, respectively the disciplinary file, shall be transmitted to the college to which the pharmacist is transferring.
