Details
The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the issuance of the transfer note, the requesting pharmacist is required to pay up to date the membership fee owed to the college they wish to leave. The transfer note will be handed, with signature, to the applicant. In the pharmacist register (https://portal.tm-c.eu/LstFarma/ColegFarm) the college to which the pharmacist has transferred will be mentioned. If a complaint has been registered with the college from which the pharmacist is transferring or a disciplinary investigation has been initiated, the complaint, or the disciplinary file, will be sent to the college to which the pharmacist is transferring.
