The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is required to pay up to date the membership fee owed to the college from which they wish to transfer. The transfer note will be handed to the applicant, against signature. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be recorded. If, at the college from which the pharmacist is transferring, a complaint has been filed against them or a disciplinary investigation has been initiated, the complaint, respectively the disciplinary file, will be sent to the college to which the pharmacist is transferring.
