The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is required to pay up to date the contributions owed to the college they wish to leave. The transfer note will be handed over, with signature, to the applicant. In the pharmacist registry (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint against the pharmacist or a disciplinary investigation has been registered at the college from which the pharmacist is transferring, the complaint or disciplinary file will be forwarded to the college to which the pharmacist is transferring.
