The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist has the obligation to pay up to date the contributions owed to the college they wish to leave. The transfer note will be handed over, under signature, to the applicant. In the pharmacists' register (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If, at the level of the college from which the pharmacist is transferring, a complaint has been registered against them or a disciplinary investigation has been initiated, the complaint or the disciplinary file will be transferred to the college to which the pharmacist is transferring.
