Details
The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is obliged to pay in full the dues owed to the college from which they wish to leave. The transfer note will be handed, under signature, to the applicant. In the pharmacists' registry (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint has been registered at the college from which the pharmacist is transferring, or if a disciplinary investigation has been initiated, the complaint or disciplinary file will be sent to the college to which the pharmacist is transferring.
