Details
The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is required to pay the membership fee owed to the college they wish to leave. The transfer note will be handed to the applicant, under signature. In the pharmacist registry (https://portal.tm-c.eu/LstFarma/ColegFarm) the college to which the pharmacist has transferred will be noted. If a complaint has been registered against the pharmacist at the college from which they are transferring, or a disciplinary investigation has been initiated, the complaint or disciplinary file will be sent to the college the pharmacist is transferring to.
