The transfer note is issued by the college where the pharmacist is registered, based on a request. Before issuing the transfer note, the requesting pharmacist is required to pay the membership fee owed to the college they wish to leave up to date. The transfer note will be handed over to the applicant under signature or sent by electronic means. In the pharmacist registry (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has transferred will be mentioned. If a complaint has been registered against the pharmacist at the college from which they are transferring, or a disciplinary investigation has been initiated, the complaint or the disciplinary file will be transmitted to the college to which the pharmacist is transferring.

Note: If the documents requested below (except for the application) have previously been submitted to the Călărași College of Pharmacists, they are not attached to the application.
