The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is obliged to pay up to date the membership fee owed to the college from which they wish to leave. The transfer note will be handed, under signature, to the applicant. In the register of pharmacists (https://portal.tm-c.eu/LstFarma/ColegFarm), the college to which the pharmacist has been transferred will be specified. If, at the level of the college from which the pharmacist is transferring, a complaint against them has been registered or disciplinary investigation has been initiated, the complaint, or the disciplinary file, will be forwarded to the college to which the pharmacist is transferring.
