The allowance is granted based on an application and a statutory declaration accompanied by supporting documents regarding the family composition, its income, and, if applicable, proof of school attendance by the dependent children.
Required documents for the file (in copy):
– application and statutory declaration;
– identity documents of the applicants;
– birth certificates of the children, their identity cards (if applicable);
– marriage certificate or family record book;
– final decision approving adoption, family placement of the minor, according to the law;
– document attesting the applicant’s status as guardian or curator;
– documents showing that a family member is attending an educational institution (school certificate for all school-age children in the family; excepted are school-age children with severe or pronounced disabilities for whom proof issued by the competent authorities showing that the child cannot be enrolled in any educational institution provided by law must be presented);
– documents showing classification as a person with severe or pronounced disability or grade I or II disability, for dependents;
– net income earned (the month prior to submitting the application) and supporting documents mentioned in the application form (certificates regarding net salary, pension slips, unemployment aid, state allowance coupon, etc.);
In the case of consensual union (cohabitation), considered a beneficiary family, documents regarding the family composition and income for the applicant’s partner must also be submitted.
