Establishing the status and social context in which the person lives for the purpose of assessment/reassessment or admission to a residential center for adults with disabilities by the Evaluation Commission for Adults with Disabilities*

The procedure describes the activities carried out to establish the status and social context in which the person requesting the social investigation necessary for evaluation / re-evaluation for classification into a degree of disability / evaluation for admission to a residential center for persons with disabilities by the Sibiu Adult Persons with Disabilities Evaluation Commission lives.

At the request of the adult with disabilities (who is registered as residing in Sibiu municipality) / his legal representative or a relative, the staff of S.A.P.D. will go to the address indicated in the application, in order to collect the data and information necessary for the preparation of the social investigation.

The signed and stamped social investigation will be handed to the applicant, who will read it to verify the authenticity of the recorded data, subsequently expressing agreement with what was read and signing to receive a copy thereof.
