The procedure aims to regulate the specific conditions for resolving requests regarding the reimbursement of the personal contribution corresponding to the value of medicines for persons from the territorial area assigned to the “Dr. I. Czihac” Iași Emergency Military Clinical Hospital, Iași, Suceava, Botoșani and Neamț counties - according to Order M110/2009 art.4, letter f.), h), j).

Persons who are unable to obtain the medicines from the pharmacies contracted with the “Dr. I. Czihac” Iași Emergency Military Clinical Hospital or cannot find all the medicines in these pharmacies may collect their prescriptions from any community pharmacy at their own expense, requesting the following documents to be attached to the reimbursement file:

- the fiscal invoice - original

- the fiscal receipt - original

- simple prescription / CNAS-type prescription (electronic) - “prescribing” component and “dispensing” component - original

In order to recover the personal contribution paid for medicines prescribed on CNAS-type (electronic) or simple prescriptions, the categories of personnel, in accordance with Order M110/2009 art.4, letter f.), h), j), shall draw up a report to the commander of the “Dr. I. Czihac” Iași Emergency Military Clinical Hospital (according to the template attached to this procedure) accompanied by the following documents:

- the fiscal invoice issued by the pharmacy that dispensed the medicines - original

- the fiscal receipt related to the fiscal invoice - original

- the simple prescription

- medical recommendation: copy of the CNAS-type prescription and the copy issued by the pharmacy

- copy of the identity document

- copy of documents proving the status of reserve/retired military personnel/pensioner (pension slip, military pensioner card, veteran card, etc.)

The report and documents are submitted to the hospital registry (CDC) by mail or in person.

The term for analysis and approval of the request regarding reimbursement of the personal contribution related to the value of medicines is 30 days from the registration of the request at the institution's registry, followed by payment in accordance with the allocated budget.
