For registration in the tax records of a cargo means of transport with a total authorized mass of over 12 tons, the form (model 2016 ITL 006) is downloaded, filled out, and electronically signed, with a qualified electronic signature.*

For the identification of individuals in relation to the local tax authority and the establishment of their fiscal domicile, the following documents should be attached electronically, as applicable: - Identity document; - Birth certificate (in the case of a minor owner who does not have an identity card, in which case the tax declaration will be completed by their legal guardian); - Tax registration certificate (the document through which the tax identification number/registration code is individualized and is used if the taxpayer does not have a personal identification number). Note: The conformity of the copies with the original is performed by the taxpayer/payer by entering the mention "according to the original," in accordance with art. 64 para. (5) of Law no. 207/2015 regarding the Fiscal Procedure Code, as amended and supplemented, and each document must be electronically signed with a qualified electronic signature.*

For the representation of taxpayers in their relationship with the local tax authority, the following documents are attached electronically: - Power of attorney and identity document of the authorized person. Note: The conformity with the original of the copies is carried out by the taxpayer/payer by inserting the mention "certified true copy", according to art. 64 para. (5) of Law no. 207/2015 regarding the Fiscal Procedure Code with subsequent amendments and additions, and each document will be electronically signed with a qualified electronic signature.*

In the case of individuals, for the registration in the fiscal records of a means of transport with an authorized total mass of over 12 tons, the following supporting documents are attached electronically, as applicable: - Identity document, - Document of acquisition of the means of transport, which may be, as applicable: Fiscal invoice Contract of sale-acquisition Exchange contract Donation deed Deed of payment in lieu Adjudication deed Court decision, final and irrevocable/final and enforceable, etc. - In the case of means of transport subject to financial leasing contracts: Financial leasing contract and/or novation/assignment contract Proof of recovery and request for cancellation, accompanied by the handover-takeover report or any other similar document, in the case of taxpayers who are lessees; - Certificate issued by the Autonomous Administration Romanian Auto Registry, certifying the suspension system for means of transport with an authorized total mass of over 12 tons - Vehicle's identity card; - Tax attestation certificate issued in the name of the seller/former user, by the specialized department of the competent local public administration authority, except for means of transport acquired from abroad. In the case of transfer of ownership, by using, as applicable, the CONTRACT OF SALE-ACQUISITION, MODEL 2016 ITL-054, the stamp applied in the upper right corner represents the equivalent of the TAX ATTESTATION CERTIFICATE (according to ORDER 1069/1578/114 from 28.07.2016)! Note: The conformity with the original of the copies shall be carried out by the taxpayer/payer by entering the note "copy conforms to the original", according to art. 64 para. (5) of Law no. 207/2015 regarding the Fiscal Procedure Code with subsequent amendments and additions, and each document shall be signed electronically with a qualified electronic signature.*
